We have also scanned in and loaded a black and white PDF version of the Eagle Scout Rank Application (No 58-728 - 1999 edition) onto our site.  It can NOT be used as the form submitted to BSA for an Eagle Scout candidate, but is provided here purely for reference purposes.  You may also try using that document to work with the templates Rick has provided.  Note also, that the pdf file is quite big (262 KB), so it will take a while to download.
